Brightspace sometimes renders attachment menus after the page is visible. Hard refresh (Ctrl+Shift+R), scroll a bit so attachments load, then open the extension again. If it shows files from a different submission, go to the next submission using the top-right arrow, then come back and reopen the extension to force Brightspace to refresh the attachment list.
